چکیده

Objective: In locally advanced non-metastatic rectal carcinoma, pre-operative radiotherapy is an acceptable alternative over post-operative radiation to improve locoregional control after radical surgery. There are two regimens of – short-course (25 Gy/5 fractions/1 week) and long-course chemoradiotherapy (CRT) (50.4 Gy/28 fractions/5.5 weeks). Our study aimed compare the pathological response, margin negative surgery rates, treatment-related acute toxicities between these approaches. Methods: Patients with histologically proven advanced, adenocarcinoma were randomized into group received followed by 7–10 days completion weeks) concurrent capecitabine 4–6 weeks radiotherapy. Histopathology reports studied in both groups for determination response tumor surgical status. All patients adjuvant chemotherapy 6 months oxaliplatin capecitabine. For assessment toxicities, examined during entire course treatment. Results: Overall (complete response+partial response) was 81.25% arm 86.66% arm. Complete rate 15% 25% Margin rates higher CRT than (90% vs. 82%), but it statistically insignificant. Radiation-induced skin reactions (less Grade 2) significantly (p=0.003). Conclusion: no significant difference concomitant terms efficacy toxicity profile. Thus, our limited resources huge patient load, can be used as CRT.

A Retrospective Analysis on Two-week Short-course Pre-operative Radiotherapy in Elderly Patients with Resectable Locally Advanced Rectal Cancer

To validate that a two-week short-course pre-operative radiotherapy regimen is feasible, safe, and effective for the management of elderly patients with locally advanced rectal cancer (LARC), we retrospectively analyzed 99 radiotherapy-naive patients ≥70 years of age with LARC. Patients received pelvic radiation therapy (3D-CRT 30Gy/10f/2w) followed by TME surgery; some patients received adjuva...

Preoperative radiotherapy of locally advanced rectal cancer: clinical outcome of short-course and long-course treatment with or without concomitant chemotherapy.

BACKGROUND Preoperative radiotherapy is considered to be standard treatment for locally advanced rectal cancer. The timing and dosage of radiotherapy with or without preoperative chemotherapy remain controversial issues. The objective of this study was to evaluate relevant clinical outcomes of two preoperative radiotherapy regimens - the short-course and -long-course radiotherapy with or withou...

Upfront systemic chemotherapy and preoperative short-course radiotherapy with delayed surgery for locally advanced rectal cancer with distant metastases

BACKGROUND Choosing the most effective approach for treating rectal cancer with mesorectal fascia (MRF) involvement or closeness and synchronous distant metastases is a current clinical challenge. The aim of this retrospective study was to determine if upfront systemic chemotherapy and short-course radiotherapy (RT) with delayed surgery enables R0 resection. METHODS Between March 2009 and Oct...

Short-course versus long-course neoadjuvant radiotherapy for lower rectal cancer: a systematic review.

OBJECTIVE To analyse the efficacy of short-course (SCRT) versus long-course radiotherapy/chemoradiation (LCRT) as a neoadjuvant modality for the management of lower rectal cancer (LRC). METHODS A systematic review of the literature was undertaken. Selected trials were analysed to generate a summative outcome. RESULTS Seven trials on the efficacy of SCRT versus LCRT as a neoadjuvant modality...

Concomitant Chemoradiation versus Accelerated Radiotherapy in Selected Cases of Locally Advanced Carcinoma Cervix – A Prospective Study

Introduction : Although concomitant chemoradiation (CTRT) is the standard of care for locally advanced cervical cancer (LACaCx), it cannot be administered safely in elderly patients or those with certain comorbidities. Aims: To evaluate the feasibility and efficacy of six fractions per week of pure accelerated external beam radiotherapy (EBRT) with standard CTRT, for selected patients with LACa...
